We are looking for experienced Occupational Therapists, Physiotherapists or Registered Nurses to join our growing rehabilitation team as Rehabilitation Case Managers.

We are particularly interested in hearing from clinicians with experience of serious injury, complex trauma, neurological conditions or complex rehabilitation needs.

This is an exciting opportunity to use your clinical expertise in a different way – combining assessment, rehabilitation planning, clinical reasoning and coordination to help people achieve the best possible recovery outcomes.

You will work remotely, managing your own caseload, while being supported by an experienced multidisciplinary team including doctors, psychologists, physiotherapists, occupational therapists and nurses.

About the role

As a Rehabilitation Case Manager, you will:

• Manage your own caseload and diary remotely

• Complete Initial Needs Assessments and rehabilitation reviews within clients’ homes

• Assess the physical, functional, psychological, social and vocational impact of injury

• Identify rehabilitation needs, priorities and potential barriers to recovery

• Develop and coordinate individualised rehabilitation plans based on clinical assessment and identified needs

• Coordinate treatment, therapy and other rehabilitation interventions and monitor progress over time

• Work collaboratively with healthcare professionals, insurers, solicitors and other stakeholders

• Provide clinical insight and recommendations to support effective rehabilitation decision-making

• Prepare clear, evidence-based reports that explain your clinical findings and recommendations

Complex injury experience

We are particularly interested in clinicians with experience supporting individuals following significant or complex injury.

Relevant experience may include:

• Major trauma and polytrauma

• Complex orthopaedic injuries and multiple fractures

• Traumatic or acquired brain injury

• Spinal or neurological injury

• Limb loss or significant functional impairment

• Complex mobility, care, equipment or home adaptation needs

• Persistent pain and its impact on function and recovery

• Psychological difficulties following serious injury

• Complex discharge and community rehabilitation

• Return-to-work or vocational rehabilitation following significant injury

You do not need experience in every area.

We are looking for clinicians with strong assessment skills, sound clinical reasoning and an understanding of how physical, psychological, social and vocational factors can interact following serious injury.

About you

You will be a qualified:

• Occupational Therapist registered with the HCPC

• Physiotherapist registered with the HCPC

• Registered Nurse registered with the NMC

You will also have:

• A minimum of 3 years post-registration clinical experience

• Experience working with patients with significant, complex or long-term clinical needs

• Strong clinical assessment and reasoning skills

• Excellent communication, organisation and report-writing skills

• The ability to consider the wider functional and rehabilitation consequences of injury

• Confidence communicating with clients, families and a range of clinical and professional stakeholders

• The ability to work independently while contributing effectively within a multidisciplinary team

Experience of major trauma, brain injury, neurological rehabilitation or other serious and complex injuries would be particularly welcomed.

Essential requirements

• Hold a full UK driving licence and have access to their own vehicle for patient assessments and reviews (travel expenses paid)

• Be willing to travel when required, including occasional longer-distance assessments (up to three hours each way) or occasional overnight stays

• Hold current HCPC or NMC registration, as appropriate to their profession

• Be UK based (we are unable to offer sponsorship)
