Hours: Full-time or Part-time (minimum 3 days per week / 21 hours)
We are looking for experienced Occupational Therapists, Physiotherapists or Registered Nurses to join our growing rehabilitation team as Rehabilitation Case Managers.
This is an exciting opportunity to use your clinical expertise in a different way – combining assessment, rehabilitation planning, clinical reasoning and coordination to help people achieve the best possible recovery outcomes.
You will work remotely, managing your own caseload, while being supported by an experienced multidisciplinary team including doctors, psychologists, physiotherapists, occupational therapists and nurses.
As a Rehabilitation Case Manager, you will:
Manage your own caseload and diary remotely
Complete Initial Needs Assessments and rehabilitation reviews within clients’ homes
Develop individualised rehabilitation plans based on clinical assessment and identified needs
Coordinate rehabilitation pathways and monitor progress over time
Work collaboratively with healthcare professionals, insurers, solicitors and other stakeholders
Provide clinical insight and recommendations to support effective rehabilitation decision-making
Produce high-quality, evidence-based reports demonstrating your clinical reasoning

We welcome applications from clinicians who are considering case management for the first time, as well as experienced rehabilitation professionals looking for a new challenge.
You will be a qualified:
Occupational Therapist registered with the HCPC
Physiotherapist registered with the HCPC
Registered Nurse (RGN) registered with the NMC
You will also have:
A minimum of 3 years post-registration clinical experience
Experience managing complex clinical presentations
Strong communication, organisation and report-writing skills
The ability to work independently while contributing effectively within a multidisciplinary team
Current knowledge and experience in areas such as orthopaedic, musculoskeletal, neurological and psychological injuries
Experience of working with individuals following brain injury or serious injury would be desirable.

Have access to their own vehicle for patient assessments and reviews (travel expenses paid)
Be willing to travel when required, including occasional longer-distance assessments or overnight stays
Hold current HCPC or NMC registration
Be UK based (we are unable to offer sponsorship)
